The Short Answer
For most patients, Botox results last between three and six months. Within that range, where you sit depends on the dose, the area treated, the strength of the muscles involved, and your individual physiology.
The standard expectation we set in clinic is a follow-up review at around the three to four month mark, with treatment intervals that lengthen for some patients with consistent use.

What is actually happening biologically
Botox is a purified form of botulinum toxin type A. When injected into a muscle, it blocks the release of acetylcholine, the chemical messenger that tells muscles to contract, at the neuromuscular junction. Without that signal, the muscle stays relaxed and the dynamic lines created by repeated contraction soften over the days that follow.
Botox is not permanent because the nerve terminals it acts on eventually grow new connections to the muscle. As those new connections mature over several months, muscle contraction returns and the original movement pattern reasserts itself. This is why Botox wears off rather than disappears all at once.
What affects how long it lasts
Treatment area is one of the biggest variables. Different areas of the face use muscles of different sizes that work at different intensities, and that affects how long the effect of Botox is visible. Heavily used muscles tend to recover faster than less active ones.
Dose matters, but not linearly. Using too few units gives a shorter duration and incomplete effect. Using more than required does not proportionally extend duration. The aim at consultation at PHI Clinic is to match the dose to your muscle mass and the strength of your movement, not to maximise units.
Other factors with reasonable evidence: heavy regular exercise (faster clearance), individual metabolic rate, smoking, and significant sun damage to the treated area. Skincare and lifestyle play a more modest role than commonly claimed.
Maintenance at PHI Clinic
For our patients we suggest a follow-up review (no charge) at 12 to 14 days after treatment to assess effect and top up if needed, then a maintenance treatment when movement starts to return. Patients on a consistent schedule often find they can extend the interval over time as the relevant muscles become less active overall.
What we will not promise
There is no permanent Botox. Any clinic claiming significantly longer duration than the three to six month range should be questioned. Individual response varies, and the most honest answer is that we cannot guarantee a specific number of weeks for any single patient.
