How long do lip fillers last, and what determines it
The product. Fillers are not interchangeable. Softer, finer gels designed for the lip and mouth area, such as Juvéderm Volbella, feel better in a mobile area but do not last as long as the firmer products used to rebuild a jawline. That is a trade-off rather than a flaw: a firm filler in a lip is exactly what makes a result look and feel artificial.
How much was placed. More product takes longer to break down, simply because there is more of it. This is one reason a 1ml treatment usually outlasts a 0.5ml one, though it is not a reason on its own to have more than your lips need.
How it was placed. Injection technique matters. Product placed deeper, at structural injection points along the lip, tends to outlast product placed superficially, and the two produce different results as well as different intervals.
Your metabolism. Some people break down hyaluronic acid noticeably faster than others, and it has very little to do with anything you can control. If filler has historically not lasted as long as you expected, that is likely why, and it is worth saying so at your consultation so it can be built into the treatment plan.
Lip movement. This is the variable nobody can change. Talking for a living, playing a wind instrument or simply being very expressive all shorten the interval, because the upper lip in particular is working constantly. Smoking shortens it too, by reducing blood flow to healing tissue.
Why lips break down faster than cheeks or jawline
Lip filler does not last as long as the same product placed in the mid-face, and this is the question most people have not thought to ask.
Two reasons.
Lips move constantly. Every conversation, every meal, every expression works the product mechanically. Filler placed over a cheekbone sits in tissue that barely moves by comparison, and holds its contour for considerably longer.
Lips are well supplied with blood. A generous blood supply means faster metabolic turnover, and hyaluronic acid is broken down by your own enzymes. The better the circulation, the quicker that happens.
So if you have had dermal fillers in the cheeks or jaw and found they lasted two years, it is not a fault in the product or the technique when your lips need attention sooner. Of all cosmetic procedures involving filler, the lips are simply the hardest place to make one last.
What it looks like as lip filler wears off
Filler does not vanish overnight, and knowing what the fade looks like is genuinely useful, because it is how you tell a normal decline from a problem.
Definition goes first. The crispness along the lip border softens before the fullness does. Lip border definition and the shape of the cupid's bow lose their edge while the lip still feels reasonably full, which is why people often notice their lipstick sitting differently before they notice anything else.
Volume goes gradually. The lip loses contour rather than deflating. Most people see it in photographs before they see it in the mirror.
Any asymmetry becomes visible again. If your lips were slightly uneven to begin with, that natural asymmetry reappears as the product clears, because filler was masking it rather than changing it. Restored symmetry is one of the first things to go.
Lip lines return. Where filler was softening fine lines around the mouth, those come back as it breaks down.
None of that is a sign anything went wrong. It is the lip shape you started with, reappearing.
What the two-week review is for
At PHI we review lip filler patients at two weeks, and it is worth understanding what that appointment is actually doing.
It is not a check that anything has gone wrong. It is the point at which lip swelling has resolved, any mild bruising has faded, and you are looking at the real result for the first time. Anything judged before then is a judgement of swelling.
At that review we can add more product if you want it. This is why we deliberately place less than most people expect at your initial appointment: filler can always be added, but removing it means dissolving it, which is a further appointment and a further wait. It is also where small asymmetries get corrected.
There is very little recovery involved in any of this, which is partly why follow-ups matter more than people assume. The review sets your baseline. Knowing exactly what your lips looked like at two weeks is what makes it obvious, months later, how much has actually gone.
When to top up your lip filler
Most people maintain their lips with a top-up once the original treatment has largely worn off, which for the majority falls somewhere around the twelve month mark.
Touch-ups are usually smaller treatments than the first one, because you are restoring a result rather than building it from scratch. Your practitioner will confirm what you need at the appointment.
What is worth avoiding is topping up too early. Filler does not disappear all at once. It breaks down gradually, so there is a long window where some product remains and the lip has not returned to its original fullness. Adding more during that window means building on what is already there, and doing it repeatedly is the most common route to a lip that looks overfilled without anyone ever having decided to overfill it.
Letting a result decline properly before your next lip filler treatment in London is how lips keep natural results over years rather than months.
If you are not sure whether you are due, the useful test is not how your lips looked the week after treatment. It is whether they now look close to how they did before you started.
Lip filler is reversible either way
One of the reasons hyaluronic acid became the standard for lip augmentation is that it can be dissolved.
An enzyme called hyaluronidase breaks it down within days, whether the reason is a complication, migration, or simply not liking the result. That is not an option with permanent fillers, which is why we do not use them.
It is worth knowing for two reasons. It means a result you are unhappy with is a temporary problem rather than a permanent one. And it means the 9 to 18 month figure is a ceiling rather than a commitment: if you decide at month three that you would rather not have it, you do not have to wait it out.
Unlike some cosmetic treatments, this one is fully undoable. We only use temporary hyaluronic acid products when you have lip fillers at our Harley Street clinic, and that is a deliberate choice rather than a limitation.
