What Smokers Lines Actually Are
Vertical lip lines are the fine vertical wrinkles running from the upper lip towards the nose. They develop through a combination of repeated muscle movement, thinning skin and a gradual loss of collagen and elastin.
Two things make this area unusually prone to lines. The skin above the top lip is some of the thinnest on the face, and the orbicularis oris muscle beneath it contracts constantly through speaking, eating and drinking. Add the natural decline in collagen from the mid-thirties onwards, and the creases stop bouncing back.
They are called smokers lines because smoking accelerates the process considerably, through both the repeated pursing action and the damage tobacco does to collagen. Many people who develop these smokers' wrinkles have never smoked at all.
What Causes Them
- Skin ageing. Collagen production falls and elastin degrades, so the skin loses the ability to recover from movement.
- Smoking. Accelerates collagen breakdown and adds a repeated pursing motion.
- Sun exposure. UV damage degrades collagen faster than anything else on this list.
- Repeated expression. Pursing, sipping through straws, and habitual lip movements.
- Volume loss. As the lip and surrounding tissue lose fullness, the skin above has less support.

Several aesthetic treatments soften these lines. The right choice depends on how set they are, because lines that only show when you purse your lips need something different from lines etched in at rest.
Dermal Fillers
For lines that are visible at rest, fine hyaluronic acid filler placed directly into the crease with fine needles is usually the most direct answer. It restores support beneath the line and softens it immediately.
This is a different technique from lip augmentation. The filler goes into the lines themselves rather than the body of the lip, using a softer product and much smaller volumes. Results typically last 12 to 24 months, depending on the product used, how much is placed and the individual. Small doses of botulinum toxin relax the orbicularis oris, reducing the pursing action that deepens the lines. This works best on dynamic wrinkles that appear with movement rather than lines already etched in.

Chemical Peels

Can Smokers Lines Go Away on Their Own?
No. Smoker lines do not fade once collagen has been lost and the crease has set. Creams and serums can improve skin quality and slow further damage, but no topical product restores volume beneath a line.
Stopping smoking is still worth doing. It will not remove existing lines, but it halts one of the main drivers and improves both skin health and the appearance of the area over time.
Preventing Smokers Lines
Prevention is more effective than correction, and the measures are unremarkable:
- Daily SPF, which matters more than any other single habit
- Not smoking
- Avoiding repeated pursing where you can
- A retinoid, if your skin tolerates it, to support collagen production
- Consistent moisturising to keep the skin barrier healthy
- Sleeping on your back rather than face down
None of this reverses established lines. All of it slows the next ones. Where lines have already formed, cosmetic treatments are the only route to visible change.
